Reports showed on Tuesday that the incident occurred last Monday, May 26, 2025, when the employee, identified only as Abayomi, a forklift operator, became stuck in a scrap grinding machine.

A casual worker at the factory, speaking anonymously due to fear of victimization, disclosed that a foreign machine operator had instructed Abayomi to remove a scrap that was jammed in the machine, unaware that the machine was still powered.

“Unfortunately, he did not know that the operator had not switched it off. As soon as he removed the stuck scrap, the machine picked up and rolled him,” the worker stated.

Other workers at the scene rushed to rescue him, but by the time he was brought out, he was already dead.

As soon as he removed the stuck scrap, the machine picked up and rolled him.

An officer from the Ogun State Police, who was privy to the incident but not authorized to speak publicly, confirmed the fatality.

Efforts to get a reaction from Quantum Steel Industry were unsuccessful, as a representative stated they were not in a position to speak with the press.

The Ogun State Police Public Relations Officer, Omolola Odutola, did not respond to inquiries as of the time of filing this report.

This latest death comes after the Ogun State Government had sealed Quantum Steel Industry on March 7, 2025, along with Xinfeng Plastics Limited, following two previous industrial accidents that resulted in one death and seven severe injuries.

At that time, Farook Akintunde, Special Adviser to the Ogun Governor on Environmental Protection Agency and Chairman of the State Taskforce on Environmental Compliance and Enforcement, stated the companies were shut down to avert future occurrences.
