Atletico struck first at the Metropolitano when Robin Le Normand rose highest to head home after 14 minutes.
Real Madrid fought back quickly, with Kylian Mbappé and Arda Güler turning the scoreline around before Alexander Sørloth levelled just before half-time.
After the break, Julian Alvarez took centre stage. The Argentine converted from the spot after Arda Güler conceded a penalty, before curling in a superb free-kick beyond Thibaut Courtois to give Atletico breathing space.
Antoine Griezmann sealed the emphatic win late on, sparking emotional celebrations from Simeone and his players.
“It’s a very special day, we knew how important this game was,” Alvarez told DAZN after the final whistle. “Each penalty is a new chance, a new challenge.”
For Xabi Alonso, it was a sobering first taste of the Madrid derby. His side had won six from six in the league but looked exposed as Atletico attacked relentlessly.
“There are no excuses, we’re hurting. It’s a derby and a deserved defeat,” the Real Madrid coach admitted.

The result means Barcelona can leapfrog their arch-rivals into top spot if they beat Real Sociedad on Sunday. Atletico, meanwhile, moved provisionally up to fourth, lifting the mood after a shaky start to the season.
