The result leaves Real Madrid struggling to maintain pressure on league leaders FC Barcelona at a decisive stage of the championship race.
Despite dominating possession and creating numerous scoring opportunities, Madrid failed to convert superiority into goals.
The decisive moment arrived in the 39th minute when Martín Satriano capitalised on space inside the penalty area to fire a powerful first-time strike into the top-right corner, silencing more than 65,000 supporters inside the Bernabéu.
Madrid responded with sustained attacking pressure. Vinícius Júnior tested goalkeeper David Soria early, while Gonzalo García missed a clear opportunity from close range.
Further efforts from Trent Alexander-Arnold and Aurélien Tchouaméni were comfortably handled by the composed Getafe goalkeeper, whose assured performance proved decisive.
Second-half substitute Rodrygo came closest to restoring parity with a header that drifted narrowly wide, before a stoppage-time attempt from Franco Mastantuono was denied as frustration mounted among the hosts.
Tensions escalated in the closing stages as several Madrid players were booked, including Vinícius Júnior and Tchouaméni.
Mastantuono was later sent off deep into added time, while Getafe were also reduced to ten men after Adrián Liso received a second yellow card.
